    I am a general Obstetrician-Gynecologist and the chair of Women’s Health services at Cheshire Medical Center/Dartmouth-Hitchcock Clinic in Keene, New Hampshire. We are an integrated practice of 5 MD’s and 5 CNM’s who attend approximately 500 births per year in a community hospital setting. We are also fortunate to have several high-quality out-of-hospital birth providers practicing in nearby communities.

    As a steering committee member of the Northern New England Perinatal Quality Improvement Network (NNEPQIN), I’ve been involved in recent efforts to improve communication and the processes around consultation and transfer of women from the home or birth center environment to the hospital, when appropriate and necessary.

    I had the good fortune to participate in the Home Birth Summit held in October of 2011. This experience reinforced my commitment to continuing a constructive dialog around the provision of care services to women and their families in the site and with the practitioner best suited to their physical, emotional, and spiritual needs.

    Finally, I am in the midst of a graduate program focused on innovative approaches to the delivery of health care in what will certainly be a radically different future landscape. I believe it is time for us to look critically, with open hearts and minds, at how and where to provide women with safe, high quality maternity care at reasonable cost. With rising rates of intervention, a shrinking physician workforce and unsustainable costs of hospital-based care systems, it is time we did some out of the box thinking!
